Trupanion sells one policy structure and refuses the levers rivals use to look cheap. Every policy carries an unlimited annual benefit and a 90 percent reimbursement rate, with no lower tiers to trim the premium. The deductible is per condition, not annual: you pay it once for a condition, then never again for that same condition. This review examines what that structure costs and which pets it actually serves.
Founded in 1999, Trupanion is one of the longest-running pet insurers in the United States, and it underwrites its own policies through American Pet Insurance Company. Its signature feature is Vet Direct Pay: in participating clinics, software settles the covered share at checkout, so you front 10 percent plus any remaining deductible instead of the whole invoice.

Pricing
How much does Trupanion cost?
Trupanion's price for our test profiles appears in the score card above, stamped with the collection month; the number updates every quarterly quote run. Across runs, Trupanion has priced highest of the 11 insurers we track. The structural reason does not change between runs: every policy carries an unlimited cap and a 90 percent rate, so Trupanion prices its ceiling into every month.
The comparison that matters here is lifetime, not monthly. A chronic diagnosis changes the math permanently: Trupanion's per-condition deductible is paid once for that condition, while an annual-reset policy charges its deductible again every year the condition is treated. Cat quotes run 35 to 40 percent below dog quotes across the market, matching the pattern in our runs.
The Coverage Map
What Trupanion covers (and what it excludes)
The Coverage Map below applies our standardized 14 items to Trupanion's single accident and illness policy. Trupanion folds more into its base policy than most rivals and sells no preventative coverage at all, so the excluded rows cluster at the routine-care end.
| Item | Status | Note |
|---|---|---|
| Accidents | Covered | |
| Illnesses | Covered | |
| Hereditary and congenital conditions | Covered | When symptoms first appear after enrollment |
| Chronic conditions | Covered | Onset after enrollment; no annual payout cap |
| Dental illness | Pending verification | Confirm current policy language |
| Routine dental cleaning | Excluded | Preventative care; no wellness coverage sold |
| Exam and consultation fees | Pending verification | Confirm current policy language |
| Prescription medications | Pending verification | Confirm scope for covered conditions |
| Prescription food and supplements | Pending verification | Confirm current policy language |
| Behavioral therapy | Pending verification | Confirm current policy language |
| Alternative therapy | Pending verification | Confirm current policy language |
| Spay and neuter | Excluded | Preventative care; no wellness coverage sold |
| Vaccinations | Excluded | Preventative care; no wellness coverage sold |
| Wellness add-on available | Excluded | Trupanion sells no wellness or preventative add-on |
The pattern to notice: every Trupanion design choice points at large, unexpected bills rather than routine costs. Preventative care is simply not the product: there is no wellness add-on to bolt on, so vaccines, cleanings, and spay or neuter costs stay fully yours. Rows marked pending are confirmed against current policy documents before this page's quarterly refresh; we do not guess them.
The clock
What are Trupanion's waiting periods and limits?
| Accident waiting period | Pending verification against current policy documents |
| Illness waiting period | Pending verification against current policy documents |
| Orthopedic conditions | Pending verification against current policy documents |
We publish waiting-period day-counts only after confirming them in current state filings, and Trupanion's are queued for that check. The industry rule applies regardless of the day-count: symptoms during any waiting period count as pre-existing afterward. Verify the dates printed on your policy documents at checkout.
Trupanion applies the standard pre-existing exclusion: conditions with symptoms or a diagnosis before enrollment, or during any waiting period, are not covered. The per-condition deductible changes the claim math rather than the exclusion rules: bill minus any remaining deductible for that condition, times 90 percent. Ask how the policy groups related conditions, because the grouping decides how many deductibles you pay over a lifetime.
Claims
How do Trupanion claims actually work?
In participating clinics, Vet Direct Pay is the whole claims story: Trupanion's software settles the covered share at checkout, so you pay your 10 percent plus any remaining deductible and never front the full invoice. Ask your clinic whether it participates before you need the answer.
Away from participating clinics, Trupanion claims run as standard reimbursements: pay the vet, submit the itemized invoice, and wait for review and payout. First claims take longer everywhere because the insurer reviews your pet's full vet records.
The carrier
Who underwrites Trupanion?
Trupanion underwrites through its own carrier, American Pet Insurance Company, rather than renting another insurer's paper. The brand dates to 1999, which gives it one of the longest underwriting track records in US pet insurance. Carrier details and state availability are listed in your policy documents; the quote flow is the availability check for your zip.
What makes it different
The per-condition deductible, explained
Most insurers reset your deductible every policy year, so a chronic condition pays a deductible annually for life. Trupanion inverts this: each condition carries its own deductible, paid once, after which that condition reimburses at 90 percent with no cap for the pet's lifetime. That is why this review keeps repeating one idea: Trupanion is priced for the pet that gets expensively sick, not the pet that stays well. A pet that never develops a costly condition pays the highest premium on our list for a structure it never used. That is the honest bet you are making, in either direction.
Fit Verdict
Should you buy Trupanion pet insurance?
Pick Trupanion for breeds prone to chronic and orthopedic conditions, owners who cannot front large invoices, and anyone who wants no payout ceiling. Skip it if your pet is low-risk and your budget is tight, or you want wellness and routine care bundled into the policy. Ask your clinic about Vet Direct Pay participation before enrolling; the feature only helps where it works.
Alternatives
Trupanion vs the alternatives
- Lemonade is the opposite trade: the lowest price on our list, configurable limits, and app-fast claims.
- Pumpkin splits the difference for high-risk breeds at mid-market prices, with no breed restrictions and no upper age limit.
- Embrace rewards claim-free years instead: its Healthy Pet Deductible drops $50 each year you do not claim.
Leaving
How do you cancel Trupanion (and what happens next)?
You can cancel Trupanion at any time, and state free-look rules give a full-refund window shortly after purchase. Line up replacement coverage before you cancel: a gap restarts waiting periods, and any condition Trupanion covered becomes pre-existing with the next insurer. That trap bites hardest here, because pets that benefit most from Trupanion usually carry conditions no new policy will touch. The safe sequence is in our guide to switching pet insurance without coverage gaps.
